CELEBRATE LIFE FEST

RELEASE OF LIABILITY AND PARTICIPATION AGREEMENT

PLEASE READ CAREFULLY — THIS AGREEMENT AFFECTS YOUR LEGAL RIGHTS

In consideration for being permitted to participate in Celebrate Life Fest (the “Event”), organized by Cornerstone Reproductive Health, and for being permitted to use the property and facilities of New Holland Church of the Nazarene, located in New Holland, Pennsylvania (the “Venue”), I agree, for myself and, if applicable, for the members of my family or other participants for whom I am legally authorized to sign, to the following terms:

1. Rules and Instructions

I agree to observe all posted rules, warnings, and safety requirements associated with the Event and the Venue. I further agree to follow all oral instructions or directions given by Cornerstone Reproductive Health, New Holland Church of the Nazarene, or their respective employees, representatives, volunteers, or agents.

2. Assumption of Risk and Release of Liability

I recognize that participation in Celebrate Life Fest and use of the Event and Venue facilities may involve certain inherent risks, including but not limited to walking, running, recreational activities, uneven surfaces, weather conditions, interaction with other participants, and other risks associated with outdoor or public events.

I voluntarily assume full responsibility for any personal injury, illness, property damage, loss, or other harm that may occur to me or, if applicable, the members of my family or other participants for whom I am legally authorized to sign.

To the fullest extent permitted by law, I hereby release, waive, discharge, and hold harmless Cornerstone Reproductive Health, New Holland Church of the Nazarene, and their respective officers, directors, employees, representatives, volunteers, agents, and event partners from claims, liabilities, damages, losses, or expenses arising out of or relating to my participation in the Event or my use of or presence upon the Venue, including claims arising from the ordinary negligence of the released parties, except to the extent such release is prohibited by law.

3. Indemnification

To the fullest extent permitted by law, I agree to indemnify and defend Cornerstone Reproductive Health, New Holland Church of the Nazarene, and their respective officers, directors, employees, representatives, volunteers, and agents against claims, causes of action, damages, judgments, costs, or expenses, including reasonable attorneys’ fees and litigation costs, arising from my negligent, reckless, or willful conduct or, where legally applicable, the conduct of any family member or participant for whom I am legally responsible.

4. Damage to Property

I agree to be financially responsible for any damage to the property or facilities of New Holland Church of the Nazarene caused by my negligent, reckless, or willful actions, or by the negligent, reckless, or willful actions of any family member or participant for whom I am legally responsible.

5. Governing Law

Any legal or equitable claim arising out of or relating to my participation in Celebrate Life Fest or my use of the Venue shall be governed by and construed in accordance with the laws of the Commonwealth of Pennsylvania.

6. Photography, Video, and Media Release

I understand that photographs, video recordings, and other forms of media may be taken during Celebrate Life Fest.

I grant Cornerstone Reproductive Health permission to photograph, record, and otherwise capture my image, likeness, voice, or participation during the Event and to use such photographs, recordings, and other media for promotional, educational, fundraising, advertising, social media, website, print, or other organizational purposes without compensation to me.

I understand that these materials may be used by Cornerstone Reproductive Health in connection with promoting its mission, programs, services, and events.

7. Participant Safety

For my own safety and the safety of other participants, I agree to exercise reasonable caution throughout the Event and to refrain from using headphones or earbuds while participating in activities where doing so could interfere with my ability to hear safety instructions, traffic, other participants, or Event personnel.

8. Acknowledgment and Agreement

By signing below, I acknowledge that:

• I have carefully read this Release of Liability and Participation Agreement.
• I understand the nature of the Event and the potential risks associated with participation.
• I voluntarily agree to the terms and conditions contained in this Agreement.
• I am signing this Agreement freely and voluntarily.
• I am legally authorized to sign on behalf of any minor participant or other individual for whom I am completing this form, if applicable.
• I understand that my electronic signature has the same intent and effect as my handwritten signature to the fullest extent permitted by applicable law.
